Description

As discussed in #216, upstream changes (as seen in the latest
hledger-suite) will remove amount4 when it's not explictly given.
Fortunately, the changes do not affect any balances or other
functionality.

This commit ensures the previous behavior, provides rule consistency and
visual consistency.

hledger-flow v0.16.0 (67f1776b69e312576e761e6a292688939462612f) appears to have resolved the account4 issue described in #216

Keeping these changes for the sake of rules consistency.
