lnchannel/lnhtlc: speed up balance calculation for recent ctns

Browse Source 
Move the balance calculation from lnchannel to lnhtlc.
Maintain a running balance in lnhtlc that is coupled with _maybe_active_htlc_ids
for practicality reasons.
